Privacy Statement for the Queensland Health Metro North Hospital Health Services Digital Colorectal Cancer Followup (DCCF) application (the app) for participants/patients and clinicians

Your personal information is protected by the Privacy Act 1988 (Cth) (Privacy Act) and CSIRO will handle your information in accordance with this Act and the National Health & Medical Research Council (NH&MRC) National Statement on Ethical Conduct in Human Research (2007, updated 2018).

Your personal information is being collected by CSIRO for the purposes of conducting the DCCF project and facilitating your access and use of the app, and related scientific research. CSIRO may collect personal information from users of the app, including project participants and partners/clinicians.

The personal information collected may vary for different individuals, and may include your name, contact details, login details, information relating to your use of the app e.g. Internet Protocol (IP) address and log/usage data.

CSIRO may also collect your sensitive information, including your health information, for the purposes outlined above.

The app is hosted by CSIRO on Google Cloud and/or Amazon (AWS) servers based in Australia.

CSIRO may disclose your information to third parties including Amazon, Google, Apple in addition to CSIRO's partner - Queensland Health Metro North Health Services (QHMNHS) - for the purposes outlined above.

Apple and Google use servers based in the United States (US) and other data centres around the world. This means that your information may be transferred outside of Australia to servers based in the US, Singapore and other countries.

With your consent, CSIRO and QHMNHS may also disclose your information to your treating doctors in the event of any findings that may be of concern to your health.

Please do not provide information about any other individual via the DCCF app without their consent.

Results from the project will be de-identified and/or aggregated by CSIRO and QHMNHS and published/presented by both entities in publicly available scientific journals and presented during conference presentations.
